Reference Check Addendum for Drivers of Automobiles, Vans and Light Trucks Back
Here is some suggested wording to use when hiring commercial drivers:
Our business considers the safe operation of vehicles critical to our success. Please complete the following so that we can help assure that you are qualified to operate our vehicles.
Then make sure the form includes:
  • Applicant's name
  • Social Security number
  • Previous employer(s) for preceding 10 years, including the employer’s name, address, employment dates, type of work performed and, if employment required driving, indicate type(s) of vehicle(s) driven (automobile, van/light truck, truck/tractor, bus, other)
This next section should be read, signed and dated by applicant:
This certifies that this form was completed by me, and that all entries on it and information in it are true and complete to the best of my knowledge. I understand that, if hired, any misrepresentation of information in this application is cause for immediate dismissal. I authorize this company to investigate my background to ascertain all information of concern to my employment history, whether same is of record or not, and release those providing such information from all liability for any damages resulting from furnishing this information.
Reference information from previous employer
To help our company evaluate this applicant’s professional ability, we request that you verify the information, as regards your company, provided by the applicant and respond to the following questions regarding your previous employee:
  1. Was employee able to get along with supervisors, employees and customers?
  2. Why did this employee leave your company? Resigned, Discharged, Laid-Off, Number of Accidents, Operator Violations, Unknown
  3. Would you re-employ this person? If not, why?
  4. Accident history: number of accidents involving employee, number considered preventable
  5. Any additional information regarding applicant
Then note, for your records:
  • Name of prior employer contacted
  • Date of inquiry
  • Inquiry made by whom
  • Signature
